If an Indian university marksheet, consolidated statement, provisional certificate or degree certificate is lost or damaged, the replacement process depends on both the document and the issuing university. There is no single national procedure requiring every student to file an FIR, publish a newspaper notice or prepare an affidavit.
Some universities allow duplicate marksheets to be requested through an online student-services portal with identity and examination details. A duplicate degree can involve much stricter documentation. IIT Bombay, for example, currently requires an FIR, newspaper publication and notarised affidavit before issuing a duplicate degree, while Anna University operates separate online services for duplicate marksheets, consolidated statements and degree certificates. JNU separately provides for duplicate provisional degrees, consolidated marksheets, semester grade reports and final degree certificates.
First identify exactly which document is missing

The distinction between an original issue and a duplicate is real. JNU's current certificate procedure separately lists a normal consolidated marksheet and a duplicate consolidated marksheet, with separate entries for a provisional degree and duplicate provisional degree.
Route 1: A semester marksheet or grade sheet is missing
A missing semester marksheet is generally handled through the university's examination or Controller of Examinations system. The university needs enough information to locate the exact examination record, which can include the student's registration or roll number, programme, semester, examination month and year, and identity details.
Anna University's current application for a duplicate grade sheet asks for the student's name, register number, date of birth, degree and branch, campus, examination month and year, contact information and details of an attached identity proof. Its Student Online Services portal separately provides a Gradesheet / Marksheet [Duplicate] service.
JNU similarly treats the semester record as its own document category. Its current university-certificate procedure provides for a duplicate copy of Semester Grade Report, separately from the consolidated marksheet and degree certificate.
Delhi University also lists Duplicate Mark Sheet as a distinct examination service and provides a separate duplicate marksheet request form.
What information should you gather before applying?
Even though requirements differ, it is useful to locate your:
university roll number, registration number or enrolment number;
programme and college or department;
semester or year for which the document is missing;
month and year of the examination;
date of birth and identity proof where requested;
copies or scans of the missing document, if you happen to have one;
other marksheets that can help identify the academic record.
These are preparation items, not a claim that every Indian university asks for all seven.
Route 2: The consolidated marksheet is missing
A consolidated marksheet normally combines academic results from multiple semesters or years into one university record. Losing it does not necessarily mean that all of the underlying semester marksheets have also become invalid or need to be reissued.
Anna University currently treats a Duplicate Consolidated Statement of Marks / Grades separately from both a duplicate individual marksheet and a transcript.
JNU makes an equally useful distinction. Its current procedure separately lists:
Consolidated Marksheet;
Duplicate of Consolidated Marksheet;
Duplicate Semester Grade Report;
Duplicate Degree Certificate.
This is evidence that "duplicate academic document" is not one generic service even within a single university.
What if you never had a consolidated marksheet?
Then your request may be for a new consolidated statement, not a duplicate.
This matters when communicating with an examination branch. "I have lost my consolidated marksheet" and "my university never issued me a consolidated marksheet and I now need one" describe two different administrative situations.
Do not label a first-time request as a duplicate merely because the underlying semester results are several years old.
Route 3: The provisional degree certificate is missing
A provisional degree is more complicated because it is intended to provide evidence of successful completion before or during the period in which the final degree certificate is unavailable.
Universities can therefore place time or status restrictions on duplicate provisional certificates.
JNU currently provides a separate service for a duplicate copy of provisional degree.
Anna University's current instructions take a different approach. They state that a duplicate provisional certificate will not be issued after the convocation has been conducted and the degree awarded. The same instructions state that verification of a provisional certificate will not be entertained after the convocation and award of the degree.
This is an important example of why a student should not assume that a lost provisional certificate can always be replaced indefinitely. If the final degree has already been awarded, the university may expect the student to use or replace the final degree instead.
Route 4: The final degree certificate is lost
A lost final degree can trigger a more formal replacement process because the degree certificate is the university's formal evidence of the award.
IIT Bombay's published duplicate-degree procedure is a clear example. It states that a duplicate degree is issued where the original degree certificate has been lost or stolen and requires the applicant to:
lodge an FIR at the nearest police station;
publish notice of the lost degree certificate in a newspaper;
provide an affidavit prepared by a Notary on non-judicial stamp paper;
submit an application to the Deputy Registrar (Academic);
attach a photocopy of the original IIT Bombay degree if available;
submit the required fee and supporting originals.
The institute's procedure also states that the duplicate must be returned for cancellation if the lost original is later recovered.
This is a valid IIT Bombay procedure. It should not be rewritten as "all Indian universities require an FIR, newspaper notice and affidavit for a duplicate degree."
Lost and damaged certificates may follow different routes
A certificate that still exists but has been torn, water-damaged, defaced or otherwise damaged is not always processed in the same way as a document that has disappeared.
Anna University explicitly separates damage and duplicate services in its certificate fee structure. Its current instructions state that damaged certificates for which correction or replacement is requested have to be sent to the Controller of Examinations along with the online application documentation so that processing can take place after the original damaged certificate is received.
That makes sense administratively: if the original still exists, the university can require it to be surrendered rather than treating the certificate as irretrievably lost.
Are an affidavit and police complaint always required?
No.
The university's published procedure determines what is required.

Anna University's declaration for duplicate certificates covers a statement of marks or grade sheet, degree certificate, provisional certificate and consolidated statement, and records that the document was lost or destroyed. It also requires the applicant to undertake to return the duplicate if the original is later recovered.
The practical lesson is to download the current form for your exact document before arranging an affidavit or visiting a police station. Otherwise you can spend time preparing evidence the university does not require, or miss evidence that it does.
Duplicate Degree vs Transcript vs Consolidated Marksheet vs DigiLocker Copy
These four records can all contain academic information, but they solve different problems.

Anna University's current Student Online Services portal demonstrates the first three distinctions directly. It lists Degree Certificate [Duplicate], Consolidated Statement of Marks / Grades [Duplicate] and Transcripts as separate services.
A DigiLocker document is not just a scan you uploaded yourself
This distinction matters if your physical document has disappeared.
DigiLocker separates Issued Documents from files that users upload to their own DigiLocker Drive. Issued Documents come electronically from an integrated issuing authority. A file uploaded by the student is simply a user-uploaded copy.
The National Academic Depository states that academic degrees, marksheets and other awards issued through DigiLocker NAD are deemed at par with original physical documents under the applicable Information Technology framework. It also states that employers and academic institutions can verify such records with the student's authorization.
UGC reinforced this position in its 12 September 2022 communication, asking higher educational institutions to accept degrees, marksheets and other educational documents available in the Issued Documents section of DigiLocker as legally valid documents.
Therefore, if your university has issued your degree or marksheet through DigiLocker/NAD, check the Issued Documents section before assuming that the loss of the paper certificate leaves you without an official academic document.
When a DigiLocker copy may not solve the entire problem
Legal validity does not mean that every application process uses the same submission channel.
A foreign university, professional regulator or credential evaluator may require academic records to be transmitted directly by the issuing institution even when the student already has a valid DigiLocker copy.
World Education Services currently states that academic transcripts used in its evaluations must be sent by the issuing academic institution through an approved electronic channel or in the required sealed format. Degree certificates, when required by a particular WES application, are handled separately.
For Indian credentials specifically, WES states that it accepts consolidated transcripts and yearly marksheets when they are issued by the degree-granting institution and transmitted according to its requirements.
So a valid digital degree and a WES-compliant transcript request answer two different questions.
Which replacement should you use for employment, higher studies or verification?

Anna University's current verification system illustrates the employment and verification distinction. Companies, consultancies and embassies can apply to the university for genuineness verification of degree certificates or provisional certificates. The existence of a certificate and the process for independently verifying it are therefore separate services.
A practical recovery sequence
If an important university document has disappeared, a reliable sequence is:
Identify the exact document. Semester marksheet, consolidated statement, provisional certificate and degree certificate are not interchangeable.
Check DigiLocker/NAD. Your institution may already have issued a legally recognised digital copy.
Open the issuing university's current examination or certificate-services page. Avoid relying on a generic affidavit template from another institution.
Choose the exact service. Duplicate marksheet, duplicate consolidated statement and duplicate degree can have separate applications.
Check whether the original is lost or merely damaged. The procedures can differ.
Collect the identifying information requested by the university. This can include registration number, examination details, identity proof and supporting academic records.
Prepare an FIR, affidavit or newspaper notice only when the applicable procedure requires it.
Check what the receiving organisation actually needs. A duplicate degree may solve an employment-verification problem but not a foreign university's requirement for an institution-sent transcript.
The safest approach is therefore to recover the same type of document that was lost unless the organisation asking for proof expressly accepts another form. A transcript does not automatically replace a degree, a newly issued consolidated statement is not necessarily a duplicate, and a student-uploaded scan in cloud storage is not the same as a university-issued DigiLocker academic award. The issuing university's current procedure determines how the replacement is obtained, while the receiving organisation determines which version it will accept.
